MID-FACE and LOWEr-Face FILLER TREATMENTS

The Collective Clinic is looking for two (2) patient models for an upcoming training event on December 11, 2025, from 1PM to 4PM for the following treatments:

  • Lower Face: Jawline, Chin, and Lip.

  • Mid-Face: Cheeks and Under-Eye.

This model call is complimentary; however, they require a $200 booking deposit, which will be refundable at the follow-up appointment.

By becoming a patient model for The Collective Clinic, you can receive top-quality skin treatments from our highly trained experts at preferred model rates. This will help you achieve optimal skin health and beauty.

Your information will be kept confidential in our model database for future training opportunities.

Patient Model Terms & FAQs

 

  • Every patient model is required to provide Media Consent for marketing and educational usage of your photos and videos.

    We may be able to crop areas of your face that are not being treated, but we cannot guarantee this.

  • Yes! We require a $200 non-refundable booking deposit, which will go towards your treatment.

  • Once you are approved to be a Patient Model, our team will contact you to discuss the preferred model terms and rates.

  • Unfortunately, patient model preferred rates are only available as a one-time opportunity unless we specifically invite you to come back as a patient model.

  • Certainly! Please submit your application and photos. You will still be required to pay the $100 non-refundable deposit.

    You will only receive the preferred rates for the model call-out days; after this, our regular pricing applies.

  • Unfortunately, patient model bookings are only available as needed. You can expect to be booked within two weeks of our model call date.

    Should you apply while we don’t have an active patient model call out, your information will be kept in our confidential model database for future training and media opportunities.

  • By choosing to use our website to communicate with our office, you understand and agree to the following:  The Internet is an open network and provides no inherent protection for confidential information. Emails and images sent over the Internet are not secure and should not be used to communicate confidential and/or health information directly to our office. Please be aware that our office cannot guarantee the confidentiality or security of any information you send to us over the Internet when using e-mail or Dropbox via our website. The Collective Clinic and its staff shall not be liable for any breach of confidentiality resulting from such use of e-mail and Dropbox via the Internet and our website. E-mail must not be the primary means of communication with our office. In particular, you must contact the office by telephone or in person about critical or time-sensitive issues.
